‘People often debate whether implants should go
behind the chest muscle or in front. The fact is that there
are advantages to both and the choice relies on the
individual’s needs and personal anatomy.’
He says that with a slightly older patient, where the skin
is looser and the breast is more ‘empty’, he prefers to place
the implants in front of the muscle so they fill out the skin
more naturally.
In the case of a woman who is starting to have the
earliest signs of sagging of the breasts, placing an implant
under the muscle can give more of a lift as well as
augmentation of the size. Also, for a slim individual with not
much fatty tissue to cover the implant it is generally
considered better to go under the muscle, he says.
Sometimes the patient needs a more than just implants.
In these cases, Dr Flynn performs a formal vertical breast-
lift to elevate the new bust line and inserts implants.
Choice of size is very individual, he says. ‘At our clinic
we offer the patient a range of sizes that would comfortably
fit their anatomy. The patient shouldn’t concentrate on cup
size or volume but more on the size and shape that is
pleasing to them.’
A high-profile implant will give greater projection and
a more raised bust line, while a standard profile implant
is flatter, he says. There are also differently shaped implants
to choose from.
When it comes to contouring the lower body to
complement the upper body proportions, liposuction can
indent the waist, smooth the tummy and streamline the
hips and thighs.
‘We concentrate primarily on trimming the waist and
bulges over the hips,’ Dr Flynn says. ‘The goal is to help
people fit their jeans or into close fitting dresses more
comfortably.’
He says that recent technical advances in liposuction
have provided an additional capacity for achieving a more
targeted, sleeker effect.
‘Lipolysis or injection lipolysis allow us to fine-tune the
result and give much better skin tightening, as both the
laser treatment and the injections cause tightening of the
collagen in the skin.’
Dr Flynn says that in his clinic liposuction is most
commonly performed without the need for a general
anaesthetic, using tumescent anaesthesia, often combined
with mild sedation, instead.
During this process, the target area is injected with fluid,
which acts as a local anaesthetic. The advantage include
faster recovery and patients are able to return to their
former active lifestyle much sooner than with general
anaesthesia. acsm

Dr John Flynn, who specialises in helping patients
achieve their ideal physique.
He says that creating this result often involves adjusting
breast size in the first instance, and secondly trimming the
waist and thighs with liposuction. ‘The two different
modalities work well to achieve a more harmonious body
balance or proportion.
‘A lot of women complain that they’re a different size on
top as compared to below, making it difficult to find clothing
that fits correctly.’
Dr Flynn approaches the task of creating balance by
looking at the upper body first. ‘We can increase the
proportions up top with breast augmentation or a breast lift
and then treat the lower section with liposuction to trim the
waist and hips,’ he says.
‘With breast surgery, every patient’s requirement is
different and for surgery to be appropriate we need to be
able to tailor the surgery to suit the individual.
